Message from the President


Dear Community,


This month marks my very last full month as president of this organization. I’ve had the distinct privilege of serving two full terms as president of CNMA, and I’m so grateful for all I’ve learned along the way. Thank you for trusting me in this role. 


Over these past five years, we’ve formed alliances with our licensed midwife colleagues at CALM, launched an abortion training workshop series (complete with grant funding from the Reproductive Health Services Corps), diversified our CNMA board of directors in a meaningful way, passed an unprecedented number of bills (including scope expansion, funding for midwifery education, and mandating implicit bias training for maternity care providers – not to mention all of the bills we passed collectively as part of the California Future of Abortion Council), partnered with CALM and CMQCC on the Community Birth Partnership Initiative, and so much more. The work has been extremely challenging, but oh so meaningful. I have been astounded by the hard work of this organization’s committee leaders – they get so much done, it’s ridiculous! They remind me every single day that a hard-working team of unpaid volunteers with a teeny tiny budget, when well-organized and passionate about their work, can be powerful.  


I look forward to passing the torch to Amita Graham on November 9th at the Annual Meeting, and I know she will be great. I hope you’ll join me in welcoming her into this role and throw all of your support behind her. And on that note… it’s time to vote for the incoming CNMA Board Members who will be working with Amita! National Midwifery Week! 

October 6-12, 2024


National Midwifery Week (#MidwiferyWeek2024) was created by ACNM to celebrate and recognize midwives and midwife-led care. It's time to display your pride in the hallmarks of midwifery that you deliver every day; this week is for you and about you.


This year's theme is "Midwives Looking to the Future." Rigorous scientific evidence demonstrates that care provided by midwives improves birth outcomes and patient experiences. The evidence also supports the importance of the integration and expansion of midwifery to the improvement of health outcomes across states and nations. The profession of midwifery is rooted in the past but essential to the future. Scaling up midwifery could help reduce adverse health outcomes, even in resource-constrained environments such as rural communities and could be implemented with successful outcomes at any stage of a country’s transition to lower maternal and newborn mortality rates. Midwifery care can be cost-effective, affordable, and sustainable. National governments should invest in deploying midwives and national health plans should have a strategy to scale up midwifery.

ACNM Leading a Midwifery Practice: How and Why


For midwifery practice leaders (and aspiring leaders). Explores components of leadership, financial management, strategic planning, staffing, quality management, and performance evaluation. Participants will receive a workbook that includes all workshop slides, example forms for processes discussed, and worksheets to outline their own next steps and action plans.

 

This is an 8-hour workshop split between two days (Sunday and Monday). This course will provide opportunities to engage an in-depth exploration of what maintaining a midwifery practice entails.

 

Health Policy Updates

Additionally, both of the bills CNMA is co-sponsoring have passed!! They will now be on the Governor’s desk for signature.  You can follow which bills have been signed and vetoed in California’s SC a  here


UCSF Closure of the Masters in Midwifery:

In addition to the SF Chronicle’s front page coverage in June of the UCSF closure of the nurse-midwifery masters program, California Health Line published an article on Aug 26th highlighting this protested move by UCSF. CNMA, along with our partners at the Future of Abortion Council, has met with leadership from the School of Nursing to advocate for the importance of maintaining a master’s option for nurse-midwives. CNMA continues to actively advocate for a master’s pathway for nurse-midwives in northern California. 
















Midwifery Access California

CNMA is the convener of Midwifery Access California, a multi-year, multi-stakeholder initiative to improve midwife-led care in Medi-Cal.

Join the CNMA Board of Directors!


CNMA Board Members are actively shaping the reproductive health landscape in California, and making huge strides in access to midwifery for people in California. The Board of Directors meet virtually on the third Wednesday evening of every month. Eligible candidates must be active members of CNMA. If you’re interested in serving on the board, please contact Paris Maloof-Bury at parisbury@gmail.com. We look forward to having you serve alongside us! 


Treasurer (11/2024-11/2027)

  • Attend monthly Board of Directors (BOD) meetings

  • Perform all duties commonly incident to and vested in the office of Treasurer of a corporation, as well as all duties delegated by the President, including, but not limited to the administration of the fiscal and financial policies of the Affiliate. 

  • Supervise the maintenance of accurate corporate books. 

  • Act as custodian of the funds of the Affiliate and as signatory to the Corporation’s bank account in order to issue the checks and drafts of the Corporation as directed by the BOD. 

  • Present itemized financial reports at each monthly meeting of the Affiliate. 

  • Maintain a list of members with ACNM and Affiliate dues currently paid. 

  • The Treasurer, in consultation with the President, is empowered to jointly approve expenditures under and up to a predetermined amount in accordance with the Standard Rules and Operating Procedures developed by the BOD. 

  • Working w/CPA re: filing taxes, reviewing books, etc.

  • Receiving funds for the annual mtg.

  • Treasurer is a voting member of the BOD.

  • 3 year term: 2 years as Treasurer, followed by one year as Immediate Past Treasurer (the “Immediate Past” role is non-voting). 


Southern California Representative (11/2024-11/2027)

  • Attend monthly Board of Directors (BOD) meetings 

  • Keep in touch with the regional chapters so the regional director knows what is going on.

  • Act as a voice for the members to the BOD. 

  • Establish two-way communication with regional chapter chairs and members to update them on affiliate issues, as well as bring issues, comments, needs and requests from regional members to the BOD. 

  • Take initiative regarding events in their region and develop relationships with the members of the region, including participation at local chapter meetings, when possible.

  • They are encouraged to be involved in fundraising efforts in their region. 

  • Take a leadership role in organizing in-district visits when it comes to grassroots advocacy/legislation.

  • Regional Reps are voting members of the BOD.

  • 3 year term : 2 years as Regional Rep, followed by one year as Immediate Past Regional Rep (the “Immediate Past” role is non-voting).


Representative from Under-Represented Group (11/2024-11/2027)

  • Attend monthly Board of Directors (BOD) meetings 

  • Ideally have a well developed understanding of the Reproductive Justice Framework

  • Be in community and connection with the members of the under-represented community/communities whose voice they are elevating. Bring awareness to the specific needs of these communities to the BOD and act as a voice for the members to the BOD 

  • Attend their local chapter meetings, when possible.

  • Take initiative to be involved in events in their region and to develop relationships with the members of the region.

  • Possibly be involved with fundraising to support CNMA aims

  • They are voting members of the BOD

  • 3 year term: 2 years as Representative from an under-represented group, followed by one year as Immediate Past Representative (the “Immediate Past” role is non-voting). 


Reproductive Justice and Antiracism Committee Chair (11/2024-11/2027)

  • Participate in monthly RJ/AR meetings

  • Attend monthly Board of Directors (BOD) meetings and advise the BOD on issues related to Reproductive Justice & Anti-Racism

  • Provide Quarterly communication with CNMA members regarding committee activity

  • Interface with American College of Nurse-Midwives and their current committees focusing on racial and reproductive justice issues. 

  • Coordinate collaboration with other professional groups on issues related to anti-racism and racism-based health disparities work 

  • RJ/AR Chair is a voting member of the BOD.

  • 3 year term: 2 years as RJAR committee chair, followed by one year as Immediate Past RJAR Committee Chair (the “Immediate Past” role is non-voting).

You are invited to Southern California's 6th Annual Reproductive Health Care Within Primary Care Conference. This conference only happens once a year and it takes place on Saturday, October 12th from 8:00 am to 5:30 pm in Fullerton, California. Participants will walk away with the knowledge of institutional reproductive justice advocacy, self-managed abortion counseling, as well as learn about vulvar disease, pelvic floor dysfunction, vasectomy and more! Ultrasound for Women’s Health Providers

October 17-20 | The Midwife Center for Birth & Women's Health 

In Pittsburgh, PA 


Elevate your expertise in women's health with our Two-Day Hands-On Ultrasound Course, tailored specifically for women’s health providers. The course begins with an essential online component, focusing on the basics of Point of Care Ultrasound (POCUS). This includes understanding the physics of ultrasound, which is vital for practical application during the hands-on segment. The online course is self-paced, can be returned to many times for review or clarification. It is the students’ course for a lifetime, including updates as they are made. You will need to complete the online portion before the event.


Then, join us at our hands-on scanning event, led by nationally certified sonographers and highly skilled faculty who are also midwives, nurse practitioners and obstetric certified nurses. The training provides a thorough breakdown of gynecological and obstetric ultrasound techniques. In gynecology, you will learn about the recognition of normal female anatomy, identification of common pelvic masses, and correct placement of IUDs.


The obstetric portion of our course covers a wide range of crucial skills. In the first trimester, it includes assessments of normal pregnancy development, gestational age determination, and the identification of complications such as ectopic pregnancies and failed pregnancies. The second and third trimesters focus on detailed fetal biometry, including assessments of fetal growth patterns and positions. You will also learn to perform and interpret Biophysical Profiles (BPP) to evaluate fetal well-being.


Our course goes beyond clinical skills, teaching participants how to master the controls (knobology) of ultrasound machines to optimize image quality and ensure effective diagnoses. Additionally, the course covers professional practice elements such as ultrasound billing and documentation.


With group sizes limited to four participants, our course ensures personalized instruction and extensive practical experience. This setting enhances learning outcomes and prepares you to confidently integrate ultrasound into your clinical practice. Join us to gain invaluable hands-on experience and become proficient in ultrasound examinations.


ACNM: The Midwife as Surgical First Assistant


This course is designed specifically for midwives and addresses the promotion of vaginal birth, shared decision-making and informed consent, and functioning as an active member of the perioperative team. Attendees learn how to perform immediate pre-op preparation of the client for surgery, and during surgery provide optimal surgical exposure, participate in tissue dissection, ensure hemostasis, perform or facilitate wound closure, and perform other intraoperative functions to assist the surgeon in carrying out a safe operation that optimizes outcomes.

ACNM: The Faucett Review: an AMCB Certification Exam

Over the course of five hours, participants will review pertinent content and test-taking strategies to prepare for the AMCB Certification exam for Certified Nurse-Midwives and Certified Midwives. On-Demand Webinars and Other Resources 

DEA Virtual Diversion Awareness Training Sessions

The Drug Enforcement Administration (DEA) will host 2 one-hour Virtual Diversion Awareness Training Sessions (VDATS) for nurse practitioners, physician assistants and practitioners on Tuesday, October 8th, 2024. There will be an a.m. session held from 11:00 a.m. to noon and a p.m. session held from 2:00 p.m. to 3:00 p.m. Eastern Standard Time (EST). Please note that both sessions will cover the same content.


These sessions will cover The Rise of Stimulants Prescribing: U.S. trends related to prescription stimulant prescribing from 2012-2022.

There is no registration fee for these sessions. Participants may only register for one (1) session.

Due to system limitations, each session will have a maximum capacity of 3,000 participants. It is recommended that you register as early as possible. Participants who do not have an active DEA registration number will need to request assistance to register via VDAT@dea.gov. Participants will need to use the same e-mail address throughout the registration process.

Once registered, participants will receive an e-mail confirmation containing a link to complete a secondary registration. Upon the completion of the secondary registration, participants will receive the link required to access their selected training session.

Transforming White Privilege



Reproductive Health Access

The CDC just released the 2024 U.S. Medical Eligibility Criteria for Contraceptive Use (U.S. MEC)and the 2024 U.S. Selected Practice Recommendations for Contraceptive Use (U.S. SPR). These documents provide current evidence-based contraception recommendations for health care providers. The goals of the U.S. MEC and the U.S. SPR are to support the provision of person-centered contraceptive counseling and services in a non-coercive manner and to remove unnecessary medical barriers to accessing and using contraception. 


The 2024 U.S. Medical Eligibility Criteria for Contraceptive Use addresses the use of specific contraceptive methods by persons who have certain personal characteristics or medical conditions. New guidance includes: 

  • New recommendations for persons with chronic kidney disease with nephrotic syndrome, on hemodialysis, or on peritoneal dialysis.  

  • Updated recommendations for persons with the following personal characteristics or medical conditions: breastfeeding, postpartum, postabortion, obesity, major surgery, deep venous thrombosis or pulmonary embolism with or without anticoagulant therapy, thrombophilia, superficial venous thrombosis, valvular heart disease, peripartum cardiomyopathy, systemic lupus erythematosus, cirrhosis, liver tumor, sickle cell disease, and solid organ transplantation. 

  • Recommendations on additional contraceptive methods, including a vaginal pH modulator and new doses or formulations of combined oral contraceptives, contraceptive patches, vaginal rings, progestin-only pills, and levonorgestrel intrauterine devices. 


The 2024 U.S. Selected Practice Recommendations for Contraceptive Use focuses on provision of contraceptive methods and management of side effects and issues related to contraceptive method use. New guidance includes: 

  • New recommendation for testosterone use and risk of pregnancy among transgender, gender diverse, and non-binary persons with a uterus. 

  • Updated recommendations for provision of medications for IUD placement and for management of bleeding irregularities during implant use.

Birth Center Membership Support Grants assist birth center applicants with up to 50% of membership fees due the American Association of Birth Centers (AABC) for (1) birth centers that are applying for membership for the first time and (2) birth centers that are due for membership renewal but experiencing a fiscal crisis. The AABCF has the exclusive authority to determine all grant guidelines, criteria, timelines and awards; and will monitor all grants.


AABC Changemakers Accreditation Grant

The Changemaker Accreditation Grant was established in honor of the founders of the birth center movement and the current and future changemakers who propel the movement forward. The grant is designed to pay up to 100% of the initial accreditation fee or an accredited birth center’s annual fees due to the Commission for the Accreditation of Birth Centers (CABC). Participants will be compensated $100 for their time. ACNM Call for Members

Leadership Development Committee (LDC) Call for Members


The LDC is putting out a call for new members and an invitation to previous members to continued service. We are seeking midwives who have served in or are now serving in leadership roles in clinical practice, academia, policy, and global health who can bring their experiential knowledge to accomplishing the committee's tasks for 2024-2025. We are seeking diverse representation from across the US and all ACNM regions.


The committee will focus on 2 tasks for the next year:


1. establishing leadership competencies for midwives in various leadership roles across the profession, using the International Confederation of Midwives principles and framework for Midwifery Leadership as a basis for ACNM competencies


2. develop curriculum or partner with an academic center for midwifery leadership and entrepreneurship training opportunities


To accomplish these goals, we ask for a time commitment of 2 hours per month and mental engagement with the committee's tasks. The committee leadership will commit to running structured, efficient meetings, and providing a year-long meeting calendar.

The Racism and Midwifery Education Committee is seeking new committee members. The goal of this committee is to provide guidance and resources to address racism within midwifery education to address the recommendations from the Access to Equity in Midwifery Education Report. These recommendations are based on a collaboration with DOME to:

 

•Enhance the capacity of midwives of color who are interested in working in academia.

•Financially compensate clinical sites and preceptors.

•Reestablish ACNM mentorship programs to connect student midwives with common interests and career trajectories.

•Develop equity in midwifery education report card for midwifery education programs.

•Create a cross-institutional support program to provide safe space for students and help them gain leadership skills.

ACNM Task Force on Financial Transparency 


As a result of a motion at the annual business meeting in May, the ACNM Board of Directors is establishing a Task Force on Financial Transparency. The Task Force will: define financial transparency for the organization; review the financial information that is currently available to members and how this information publicized; determine what additional information is desired by members, and the frequency and methods of dissemination; and make recommendations to the Board to improve processes in financial transparency. Findings of the Task Force will be available to all members.

The Task Force will be facilitated by the ACNM treasurer, Susan DeJoy, and will consist of 12 members: 2 ACNM Board members, one ACNM staff member, 2 affiliate leaders, and 7 ACNM active members. The Task Force will meet monthly via zoom for approximately six months.

Affiliate leaders who wish to join the task force should submit a statement of interest to treasurer@acnm.org by July 30. Questions should be directed to treasurer@acnm.org.

Parent and Caregiver Handbook for Sickle Cell


The California Department of Public Health’s Genetic Disease Screening Program, UCSF Benioff Children’s Hospital Oakland Sickle Cell Center of Excellence, and the Sickle Cell Disease Foundation are pleased to announce the release of the Parent and Caregiver Handbook for Sickle Cell – a revision of the A Parent’s Handbook for Sickle Cell Disease, Part I and II. The handbook is a comprehensive reference manual and covers birth to age 18 years.

 

The handbook was written by physicians and other medical providers at UCSF Benioff Children’s Hospital Oakland Sickle Cell Center of Excellence with input from families. Parent and caregiver focus groups had requested the addition of concrete information on disease management and navigating the health care system in this edition. The 2024 Handbook edition also includes an updated research and treatments chapter and a new chapter on empowering families when they face bias and discrimination in health care.

The California PNS order form is necessary for specimen processing. Except Kaiser Permanente California PNS providers, all California PNS samples must be 

accompanied by a cfDNA order confirmation sheet printed from the CalGenetic Portal or CDPH Form 4094 (09/09/2022).

As a reminder, all specimens for Revvity Omics Vanadis cfDNA tests must be sent to

Allied Labs for processing. Specimens sent to other labs may delay screening results

and will require your patient to be drawn again if they wish to have testing through the   CA PNS Program. The address for Allied Labs is:

Reduce the need for patient redraws


The California PNS order form is necessary for specimen processing. Except Kaiser Permanente California PNS providers, all California PNS samples must be accompanied by a cfDNA order confirmation sheet printed from the CalGenetic Portal or CDPH Form 4094 (09/09/2022).

For instance, even if your patient presents paperwork that includes a Quest Diagnostic testing code or Natera “Vasistera” the specimen may be rejected by the CA PNS Program due to insufficient participant consent. This situation would require your patient to undergo another blood draw if they wish to continue screening through the California PNS Program.


Order screening through EMR integration with SIS 


Contact the PNS program to learn more about integrating your institution's electronic medical record (EMR) system with the California Department of Public Health (CDPH) to place Prenatal Screening Program (PNS) orders directly through your EMR.


Stationary orientation for printing


Double check stationery orientation when placing them into you printer. We have TRFs that have been entered into the printer backwards. The useful adhesives used to place the State barcodes onto the vials and TRFs are no longer functional, and it renders the barcodes useless.

Avoid writing on the TRF after it has been printed. The edits will not be incorporated

during lab accessioning. This could lead to delays in result turnaround time since the new data may require coordination verification.


Natera provides additional atypical cfDNA result consultation


In addition to Quest Diagnostics, Natera, Inc. now provides additional consultation to the CA Prenatal Screening (PNS) Program providers regarding atypical cell-free DNA (cfDNA) results produced by their laboratory. CA PNS Program atypical findings results from Natera will include the lab comment: “Additional information about thissample may be available by contacting a Natera genetic counselor at 650-646-9058.”Clinicians are welcome to call Natera for additional result details.


Updated PNS Patient Booklets 


The updated PNS patient booklets are now available for downloading online. The revised booklets include information about the newly added sex chromosome aneuploidies (SCAs) or X and Y chromosome variation screenings.

The updated PNS Provider Handbook is now available for viewing and downloading on the PNS website. This version includes important information on the addition of the most common sex chromosome aneuploidies to the cfDNA screening panel: Turner syndrome, Klinefelter syndrome, XYY syndrome, and Trisomy X. Although SCA screening is similar to the other genetic disorders screened through the PNS program, there are important differences to be aware of. The updated PNS Provider Handbook will only be available online as a PDF on the website Education Resources For Healthcare Providers.

Outcome of Pregnancy Report Porta


The PNS Program is creating a new “Outcome of Pregnancy Report” portal for prenatal care providers. The PNS Program expects to have the “Outcome of Pregnancy Report” portal up and running by around the end of 2024 at CalGenetic Portal (https://calgenetic.cdph.ca.gov).

Prenatal care providers will be able to complete and submit the “Confidential Report of an Outcome of Pregnancy” survey directly through the online portal. The survey collects data on pregnancy and birth outcome, chromosomal abnormalities, neural tube defects, and other structural defects. It is sent out 60 days after the estimated delivery date for all screen positive pregnancies and a 1% random sample of screen negative pregnancies.

The main objective of the survey is to evaluate the effectiveness of the policies and practices adopted by the California Prenatal Screening Program, as required by the California Code of Regulations (Title 17, Section 6527).

Midwifery Learning Collaborative Highlights Video Now Available! 


In 2021, the Institute for Medicaid Innovation (IMI) launched the national Medicaid and Midwifery Learning Collaborative, an intensive, three-year initiative, led by IMI and funded by the W.K. Kellogg Foundation.

Five state-based teams - Arizona, California, Kentucky, Michigan, and Washington - were selected from a competitive application process. Each team received technical assistance and guidance from IMI and a dedicated national advisory committee to develop sustainable initiatives to advance midwifery-led models of care for those enrolled in Medicaid. Each team consisted of leaders and innovators from four key groups: state Medicaid agencies, Medicaid health plans, community-based organizations, and provider groups, including midwives of all credentials.


More than 60 midwifery leaders convened in Detroit in June to celebrate progress.
